Please include the destination, dates, and reason for the activity, along with the roster of students, staff, and faculty who will be participating on the program. Once completed, please remember to hit submit.

After this information has been submitted, the student participants are required to complete the individual travel registry, which includes:

• Arrival and departure dates of program
• Location of program
• Emergency contact information
• Source of funding
• On-site contact
• Local address
• Personal phone number
• Activity on site
• Method of transportation to site
• Medical insurance information
• Acknowledgement of risk and responsibility

Students participating on an off-campus program may also be responsible for completing the Confidential Physician's Report and submitting the completed form to the group leader. If the group is traveling internationally, each student is also required to register for CISI traveler's insurance.
